I (the parent/guardian, if the player is below 18 years old), by electronically signing this enrollment form, hereby consent to our child's (or our team's) participation in the Dream Big Basketball programs. Our child (or the players on our team) is in good health and can participate in all activities. The parent releases all Dream Big Basketball programs and/or any officers, owner/lessor of facilities (including but not limited to Dream Big Basketball Academy), partners (including but not limited to Dream Big Development LLC), members, directors, coaches, employees, servants, sponsors, agents, licensees and assigns of any of the foregoing, from and against any and all suits, awards, claims, damages, liabilities, costs and expenses (including reasonable attorney fees and related costs) arising out of injury or damages to my child in connection with his /her participation in any Dream Big Basketball programs.

I hereby authorize Dream Big Basketball to act for me according to their best judgment in any medical emergency situations.
I, by signing this enrollment form, grants permission to Dream Big Basketball (and it's partners/sponsors) to utilize the signer's child's images, likeness, actions and statements in any live or recorded audio, video, or photographic display or other transmission, exhibition, publication or reproduction made of, or at, the Event in any medium or context without further authorization or compensation. I give my permission to receive text messages and emails. Your private information will not be sold/given to third party.
All deposits are non-refundable, and no refunds will be given for cancellation of an event by participants if Dream Big Basketball is notified 12 hours or less before said event is set to start. If it's more than 12hours before an event when cancelling a credit will be issued to your Acuity online account (or Oasys if that's what you used to register). This credit can be used towards any future program. No expiration date. No refunds will be given for missed days, classes, trainings, or any other missed time of an event. If a class does not meet the minimum number of registrations, a full refund will be issued.
I understand that mine or my child’s place in an event is not reserved until I have either paid the full price or the non-refundable deposit for said event.
